NOT ACCESS THIS INTERNET PORTAL. Your Health Information and the content of the Portal The Episcopal Health System San Lucas has created this portal, as we have requested and authorized by the Federal Law "Health Information
Technology for Economic and Clinical Act of 2009", known as HITECH by its initials in English. It is our duty to give access and disclose the patient your health information in this Portal, applying the measures and
standards stipulated by the HIPAA Security Rule. You can use the health information (such as health history, allergies, medications, results and demographic and clinical data) that has been provided by the Episcopal
Health System San Lucas through the Portal to be revised, downloaded, and transmitted, always and when:(1) do not delete, do not alter, do not destroy any information or content, (2) use this information only for your
personal use and continuity of health care of the patient and (3) do not make any representations or warranties additional to those referred to here related to your health information and the content of the Portal.
In line with what is required by law we reserve our right to determine that your health information will be disclosed in this portal. Therefore, not all the information about your health that consists in our records
will be available for access through this Portal. Similarly, some of the information provided as results of laboratories and studies, do not constitute a formal outcome consistent with the regulations of Puerto Rico,
you should get your original result and official directly from the ease that offered the service. You understand that the health information presented on this site may contain errors and/or be under review by the Episcopal
Health System San Lucas or regulatory agencies. Therefore, we urge you to consult with your doctor before making use of your health information presented in the Portal. You may contact the Department of Health Information

Health System San Lucas has established a rigorous process of registration in the Portal to authenticate you and ensure that the health information is only disclosed to you. The use of the Portal requires you to open
an account of access only (user name and password) for your exclusive use. The account passwords for access to the Portal are encrypted and URLs are re-written. In addition, the Portal account with security features

Rules (Commercial Arbitration Rules) current and must be clarified in the Commonwealth of Puerto Rico. The arbitration shall be governed by the United States Arbitration Act, 9 U.S.C. §§ 1-16 excluding any provision
of the law of the State which is inconsistent or which produces a different result. A unique and singular mediator shall determine the claim of the Parties and take a final decision in accordance with the applicable
law. A strict confidentiality shall govern any act of arbitration, and all the information submitted to the mediator by the parties and to the decision or concession granted by the mediator. These terms should not limit
any obligation of a party to defend, indemnify or declare innocent to another party against legal acts or other claims, losses, damages, or costs. The procedures specified in this Section shall be the exclusive procedures
for the resolution of disputes against the parties arising out of or related to the use of the Portal; provided, however, that a party may request temporary remedies in court to maintain the status quo or to protect
assets or property until the arbitration shall be initiated and the mediator selected has had the opportunity to resolve the request of the temporary remedy. It is required that each party to continue carrying out its
obligations under these Terms and Conditions of use while the final resolution is pending on a dispute arising out of or relating to these Terms and Conditions of Use of the website, unless you do this is impossible
