If you encounter problems when creating your account follow these tips:
Enter your last name and mother's maiden name (second last name).
Enter your name.
Enter your birthday (Month/Day/Year).
Enter the medical record number given to you which starts with any of the (ML, MG, MT) characters followed by numbers. The entirety of it must be entered.
Confirm medical record number by repeating the number entered in the previous field.
